WEDNESDAY, July 25 (HealthDay News) -- Rates of esophageal cancer have surged due to a lack of awareness about what causes the disease and how it can be prevented, experts say.

The esophagus is the muscular tube that carries food and liquid from the mouth to the stomach. There were six times as many cases of esophageal cancer in 2001 as there were in 1975, according to a team from the University of California, Los Angeles. The researchers noted that one key way people can reduce their risk for the disease is by managing heartburn and acid reflux, often called gastroesophageal reflux disease, or GERD.

"Obesity and poor diet have spiked the numbers suffering from acid reflux," Dr. V. Raman Muthusamy, associate clinical professor of medicine and endoscopy director at the UCLA Center for Esophageal Disorders, said in a university news release.

If left untreated, GERD can cause stomach acid to wash repeatedly into the esophagus, causing changes in the tissue lining. This condition is called Barrett's esophagus, and people diagnosed with Barrett's may be up to 40 times more likely to develop esophageal cancer, the UCLA experts explained.

Complicating matters, people with esophageal cancer may not experience any symptoms other than heartburn, which could prevent early detection of the disease, said Muthusamy and his colleague Dr. Kevin Ghassemi, clinical programs director at the UCLA Center for Esophageal Disorders.

"Early identification, treatment and management of changes in the esophageal lining are critical to catching problems early," Ghassemi said in the news release.

To help people know when to be concerned about acid reflux or heartburn and reduce the risks associated with the condition, Muthusamy and Ghassemi offered the following tips:

• Anyone experiencing heartburn more than once a week should visit their doctor to manage the condition.
• Lose extra pounds. Being overweight can make acid reflux and heartburn worse.
• Avoid eating too much at one time, and keep upright after eating. Reclining with a full stomach can make symptoms worse.
• Engage in light physical activity after eating; exercise can help digestion.
• Anyone who takes medications for acid reflux -- such as Prilosec, Nexium, Prevacid, Zantac or Pepcid -- should take their medication regularly to reduce the level of acid in their stomach.
• Get screened for esophageal cancer. White men aged 50 or older who have been affected by acid reflux for more than 10 to 15 years should consider being screened for Barrett's esophagus. If caught early, the changes in the esophagus lining can be treated.
• Don't smoke.
• Avoid caffeine, alcohol and fatty foods, which can increase the risk for acid reflux.
• People taking certain heart and blood-pressure drugs, such as calcium channel blockers and nitrates, may be at greater risk for acid reflux. These patients should discuss their risk factors and treatment options with their doctor.
• Don't wear tight-fitting clothes.

The American Cancer Society estimates that there will be nearly 17,500 new cases of esophageal cancer in the United States in 2012, and more than 15,000 deaths from the disease.

The simple answer is Yes!? The more complicated reason is: why?

On June 15, 2012 President Obama made his historic announcement that certain young people who came to the U.S. (generally referred to as" DREAMers") can apply for deferred action for a 2 year period and obtain legal work authorization.? To qualify the applicant must:

1. Have come to the United States under the age of sixteen;
2. Have continuously resided in the United States for at least five years preceding June 15, 2012 and are present in the United States on June 15, 2012;
3. Currently be in school, have graduated from high school, have obtained a general education development certificate, or are honorably discharged veterans of the Coast Guard or Armed Forces of the United States;
4. Have not been convicted of a felony offense, a significant misdemeanor offense, multiple misdemeanor offenses, or otherwise pose a threat to national security or public safety;
5. Not be above the age of thirty.

Following the announcement, there was a great deal of speculation and confusion about what the application would look like, how difficult or complex it will be to complete, and whether or not an applicant would need legal representation to successfully apply.? These questions proved impossible to answer as very little information was provided by the Department of Homeland Security, who merely indicated that they would begin processing applications on August 15, 2012.? The lack of information, however, didn't stop enterprising individuals--both attorneys and non-attorneys-- from advertising to potential applicants and even to accept money to prepare forms that didn't even exist, making claims about "the new law", "being first in line", etc.? The sensible and prudent attorney was advising potential applicants to hold off until more details emerged and counseled them to begin to assemble the paperwork that would support their eligibility.?

However, in recent days more information is beginning to come to the surface and with only a couple of weeks until the August 15th date, things are getting a little clearer.?

Filing process.? While the actual Form itself is not yet published, the information necessary to complete the form can be deduced from the initial June 15th memo.? Examples of these documents can be found at our sister site: www.dhsdeferredaction.com.? There already exist several forms that ask for the information, including the G-325, I-821, and the I-765.? However, since the application would seem to be more document driven than simply information driven, it seems certain that any application would must include a significant amount of documentary evidence.? According to a USCIS source, the applications would be sent to a central lockbox (likely Phoenix or Chicago) before being routed to a USCIS service center.? It is estimated that between 800,000 and 1.4 million (and growing) applications will be filed.? It would take 2-3 months for receipt notices to be generated and biometric notices would following in another 1-2 months.? After that, the processing time for the DA form would be anybody's guess and interviews may be scheduled in the local district office if further information becomes necessary (particular criminal issues).? Following that the employment authorization would be processed.? According to the USCIS source, it is likely that RFE's (requests for evidence) would be issued in most cases. ? It is important to understand that if a case is denied, THERE IS NO APPEAL!!!

Filing fees.? In keeping with DHS policy that the costs of the application process be covered by the filing fees so as not to burden U.S. taxpayers, fees will be charged.? The biometrics fee is currently $85.? The employment authorization fee is $380.? Speculation is that because there is insufficient time to conduct a fee impact study there will be no further intial fee beyond the $465.? A story in U.S. News and World Report (http://www.usnews.com/news/us/articles/2012/07/24/dhs-deportation-proposal-could-cost-585-million) cites an internal DHS document that mentions a $465 paperwork fee, so it's looking more and more like the fee would be $465.

Use of documents for enforcement purposes.? Here is a potential problem.? This program has been identified as a DHS program, not a USCIS or ICE program.? In a recent stakeholder meeting one USCIS official indicated that any information filed under this program becomes DHS property.? This is unlike the Legalization program of the 1980's where there were specific shields in place to protect against using the information for enforcement.? This is clearly not the case here!? An example of how this could become a problem is as simple as an applicant indicating that they worked with a social security number (which may have even been their own).? On inquiry the person discloses that they told their employer in the I-9 process that they were a U.S. citizen, or provided someone else's U.S. birth certificate.? Under current law, the immigration consequences could be catastrophic.? Of possibly more concern is disclosing the immigration status and identifying information including addresses of parents.? It is certain that they CAN use this information, it is unclear if they would, or under what circumstances.? Conceivable if there are outstanding removal orders it may be of more interest to ICE.

The need for an attorney.? It is the practice of this attorney and most ethical ones that I am familiar with to charge for consultations.? That is because then we can use that time to honestly and objectively advise a client as to their particular problem without having to worry about how we can pay our own obligations at the end of the day.? I have always believed that a lawyer who provides free consultation either has a large trust fund behind him/her or will use that time to explain why the potential client needs their services (whether they really do or not).? I will frequently tell people that their case is straightforward enough that if everything they have told me is the complete truth, they probably won't need an attorney because after having represented thousands of clients over 25 years, I have a pretty good idea what makes a clean case.

One factor I consider in advising whether or not a client can self-represent is how difficult it is to correct a later problem.? Because in most applications there is an appeal process, many if not most problems can be fixed. Because in these cases, there will be no appeal mechanism, the application must be as complete and correct as possible.? Yet the application should be careful not to reveal too much information that later may be used in enforcement efforts.

In additional to some complex filing procedures mentioned above, there is still so much to be decided, including what exactly will constitute a "significant misdemeanor offense" or a "threat to national security or public safety" (which will make an applicant ineligible for deferred action), applicants need to have any criminal activity carefully reviewed in light of current immigration laws and standards, not some vague community standard or someone's reasonable hunch. The Department of Homeland Security is notorious for their expansive definitions of criminal behavior, including recently finding that tattoos make a person a threat to public safety because they may look like gang tattoos.

No one, and I mean no one, can say what will happen to this program in the future.? DHS has repeatedly said that deferred action is not a "benefit" meaning to me that even if an individual has been granted deferred action, they could still take it away during or after the two years. ? DHS refuses to indicate whether or not a denied or expired applicant will be placed into removal proceedings, instead they simply refer to a series of confusing and self-conflicting memoranda on prosecutorial discretion which even they privately admit is open to wide interpretation. ?

One thing is certain:? the information provided incident to this application will be digitalized and then becomes available not only to USCIS, but also to ICE and CBP.? Whatever a person tells DHS will be used for whatever purposes they deem proper, including enforcement. This warning is not meant to discourage applicants from applying, but rather to exercise extreme caution in providing evidence and information.? Extreme caution is not exercised by guessing, or by consulting a notario, a personal injury attorney, a family friend, a consulate, or even a community activist.? It is not enough to be well-intended!

The future well being of an applicant and possibly that of his family necessitates that the applicant consult with and retain the services of an experienced immigration attorney.? It should happen now, even before the August 15th acceptance date because there will be a rush of filing and the later a case gets filed, the later it will be adjudicated.? Attorneys will tend to charge whatever the market will bear.? Some attorneys are charging hourly rates, others are charging a minimum initial retainer to consult with and assemble documents with the understanding that more money may be required later, depending on what the final process looks like, or if there is an interview.? Still others are quoting flat fees for the entire process (which is difficult if you don't know what that process is).? Lawyers (unlike notarios or immigration consultants) in most jurisdictions are subject to regulation and cannot charge more than can be justified by the time and complexity involved in the case.

In closing, in this author's opinion, an applicant should quickly obtain the services of a competent, experienced attorney.? At a minimum, the attorney should be a member of AILA (the American Immigration Lawyers Association), and should be open about the length of time in immigration practice and the numbers of cases they have processed.? Unprecedented Greenland ice sheet surface melt

ScienceDaily (July 24, 2012) ? For several days this month, Greenland's surface ice cover melted over a larger area than at any time in more than 30 years of satellite observations. Nearly the entire ice cover of Greenland, from its thin, low-lying coastal edges to its two-mile-thick center, experienced some degree of melting at its surface, according to measurements from three independent satellites analyzed by NASA and university scientists.

On average in the summer, about half of the surface of Greenland's ice sheet naturally melts. At high elevations, most of that melt water quickly refreezes in place. Near the coast, some of the melt water is retained by the ice sheet and the rest is lost to the ocean. But this year the extent of ice melting at or near the surface jumped dramatically. According to satellite data, an estimated 97 percent of the ice sheet surface thawed at some point in mid-July.

Researchers have not yet determined whether this extensive melt event will affect the overall volume of ice loss this summer and contribute to sea level rise.

"The Greenland ice sheet is a vast area with a varied history of change. This event, combined with other natural but uncommon phenomena, such as the large calving event last week on Petermann Glacier, are part of a complex story," said Tom Wagner, NASA's cryosphere program manager in Washington. "Satellite observations are helping us understand how events like these may relate to one another as well as to the broader climate system."

Son Nghiem of NASA's Jet Propulsion Laboratory in Pasadena, Calif., was analyzing radar data from the Indian Space Research Organisation's (ISRO) Oceansat-2 satellite last week when he noticed that most of Greenland appeared to have undergone surface melting on July 12. Nghiem said, "This was so extraordinary that at first I questioned the result: was this real or was it due to a data error?"

Nghiem consulted with Dorothy Hall at NASA's Goddard Space Flight Center in Greenbelt, Md. Hall studies the surface temperature of Greenland using the Moderate-resolution Imaging Spectroradiometer (MODIS) on NASA's Terra and Aqua satellites. She confirmed that MODIS showed unusually high temperatures and that melt was extensive over the ice sheet surface.

Thomas Mote, a climatologist at the University of Georgia, Athens, Ga; and Marco Tedesco of City University of New York also confirmed the melt seen by Oceansat-2 and MODIS with passive-microwave satellite data from the Special Sensor Microwave Imager/Sounder on a U.S. Air Force meteorological satellite.

The melting spread quickly. Melt maps derived from the three satellites showed that on July 8, about 40 percent of the ice sheet's surface had melted. By July 12, 97 percent had melted.

This extreme melt event coincided with an unusually strong ridge of warm air, or a heat dome, over Greenland. The ridge was one of a series that has dominated Greenland's weather since the end of May. "Each successive ridge has been stronger than the previous one," said Mote. This latest heat dome started to move over Greenland on July 8, and then parked itself over the ice sheet about three days later. By July 16, it had begun to dissipate.

Even the area around Summit Station in central Greenland, which at 2 miles above sea level is near the highest point of the ice sheet, showed signs of melting. Such pronounced melting at Summit and across the ice sheet has not occurred since 1889, according to ice cores analyzed by Kaitlin Keegan at Dartmouth College in Hanover, N.H. A National Oceanic and Atmospheric Administration weather station at Summit confirmed air temperatures hovered above or within a degree of freezing for several hours July 11-12.

"Ice cores from Summit show that melting events of this type occur about once every 150 years on average. With the last one happening in 1889, this event is right on time," says Lora Koenig, a Goddard glaciologist and a member of the research team analyzing the satellite data. "But if we continue to observe melting events like this in upcoming years, it will be worrisome."

Nghiem's finding while analyzing Oceansat-2 data was the kind of benefit that NASA and ISRO had hoped to stimulate when they signed an agreement in March 2012 to cooperate on Oceansat-2 by sharing data.

The other happy vegan and myself take our food very seriously. ?We also take our work pretty seriously as well. ?We tend not to socialize much during busy times, neither one of us goes out much. ?What we have in common at all times is our love of food. ?Granted I cook the majority of the food for ?us (and all the food for the guests), but it matters not, we both love to eat. ?That mostly explains my commitment to running and boot camp classes. ?When we get busy, we can get a bit crabby with each other. ?This includes issues with food. ?As I mentioned at the outset, we take our food very seriously.

As if the tea situation weren't bad enough, the next morning when I woke up and began rattling the pots and pans, I noticed that my leftovers were not where I left them in the fridge. ?I rummaged around thinking they were misplaced (we have a very large commercial fridge), but I couldn't locate my most favorite tempeh reuben leftovers. ?A thought almost too hard to believe crossed my mind, but I had to ask. ?"Did you eat the rest of my sandwich?" I said to the other happy vegan in the dark before I even had the muffin ingredients mixed in the bowl for breakfast. ?He admitted it. ?He ate my sandwich. ?My tempeh reuben leftovers. ?And then he said "it was only a little bit left."

When he returned from his night out, I woke up because I heard the door open and close. I asked him how was the movie. ?But, there was a more important question on his mind. ?He said "how was your vacation?" It was not until that moment I could confirm he knew I needed a break, as neither of us said it.?

I couldn't help but squeak with glee "it was GREAT!"

This accomplishment opens the door for creating more complicated virtual organisms, potentially accelerating research and making it possible for bioengineers to use computers to design organisms, said lead researcher Markus Covert, a professor of bioengineering at Stanford University.

Using these computer-model organisms, researchers could test out ideas and compare their results to what is seen in living things. In particular, these virtual "organisms" could help them probe the complexity of many biological phenomena, Covert said.

"The fact is, we won't be able to understand how those things interact together unless we use a rational, computer-based approach."

To create the model organism, Covert and colleagues combed more than 900 sources of information about the single-celled M. genitalium, which can cause inflammation of the urethra and the cervix as well as pelvic inflammatory disease. They built a model of the organism's genetic structures and machinery for each of 28 cellular processes, such as the replication of DNA (deoxyribonucleic acid, the code that makes up genes) and cell division. They then put these models together to simulate a whole cell.

Covert and colleagues have live M. genitalium microbes in the lab, which they used to test the model's predictions.

Their results were outlined Friday in the journal Cell. In addition to Covert, authors of the study, titled "A Whole Cell Computational Model Predicts Phenotype From Genotype," include Jonathan Karr, Jayodita Sanghvi, Derek Macklin, Miriam Gutschow, Jared Jacobs, Benjamin Bolival, Nacyria Assad-Garcia and John Glass.

Let?s start with the bad news. Climate change, as you might expect, could soon create trouble for snow leopards living in the Himalayas. According to a study conducted by scientists from the World Wildlife Fund (WWF) and published in the June 2012 issue of Biological Conservation, warming temperatures and wetter conditions could cause trees to grow farther uphill than they do today. Because snow leopards live in mountainous regions above the tree line, this shift could result in the animals losing as much as 30 percent of their current habitat?from 20,000 square kilometers to 11,700 square kilometers. The cats can only climb so much higher on the mountains before they run out of space and suitable temperatures, so the advancing tree lines will not only restrict their current habitat but potentially put them into further conflict with humans and livestock.

Leopards are already facing multiple threats from poaching, overgrazing of their habitat by farm animals and other human?animal conflicts. ?As grazing intensifies and the leopards? natural prey decline, they could begin preying more heavily on livestock, resulting in increased retaliatory killings,? co-author Rinjan Shrestha, the WWF?s snow leopard expert in Nepal, said in a prepared statement.

The study?s authors recommend securing additional protected habitat for the snow leopards, especially because individual animals have very wide territories (up to about 100 square kilometers each) and they enjoy their solitude. Males have a tendency to aggressively defend their home turf from other males, but they do allow two or three females to live within their territories.

A new study from the Wildlife Conservation Society (WCS) illustrates this nicely. Once the animals woke up, they quickly went on walkabout. The first leopard, captured and released on May 27, traveled 125 kilometers; the second, collared and released on June 8, traversed 153 kilometers by the time the WCS put out a press release about the research project on June 17. The animals? journeys are being documented for an upcoming Nat Geo WILD documentary, but the information will also help researchers to ?learn more about the range, behavior, movements and habitat used by snow leopards,? according to Peter Zahler, WCS Asia Program deputy director. ?This information in turn will help us in our partnership with the Afghan government and local communities to design protected areas and management strategies to optimize the conservation of this big cat.?

Den mates
Snow leopards are only solitary after they get to adulthood, and they don?t always live all that far away from humans. Researchers from Panthera and the Snow Leopard Trust this week released video of two female snow leopards and her cubs in Mongolia?s Tost Mountains. The animals were living in a den located just behind a rough, man-made wall. Check it out:

The team of researchers entered the den while the mother was away. They carefully measured, weighed and photographed the cubs as well as monitored the family unit for several days. The data provided what the team called a ?rare glimpse? into the early life of snow leopards, including when and where they give birth and the size of their litters. ?Knowledge about the first days and weeks of life is vital to our understanding of how big cat populations work and how likely it is for a newborn to reach adulthood and contribute to a healthy population,? Howard Quigley, Panthera?s executive director for its jaguar and cougar programs, said in a prepared release.

Cardiac cubs
Our final snow leopard news item comes not from Asia but from Woodland Park Zoo in Seattle. Two female leopard cubs made their video debut on July 12, one month after their male littermate had to be euthanized due to severe heart defects. All three cubs were also born with eye and eyelid defects known as multiple ocular coloboma, passed on by their father. Similar genetic problems have been observed in other zoo animals, because only about 550 slow leopards live and breed in captivity worldwide. The two remaining cubs recently received cardiac ultrasounds that revealed ?mild functional deficiencies in several valves,? but the zoo?s director of animal health said their hearts are working fine. Because of their vision problems, though, it?s unclear if or when they will go on public display. For now, you can see them here:

Private special education schools would see a massive payday if the governor of New York signs legislation currently sitting on ?his desk.

School districts are begging Gov. Cuomo to veto a bill proposing changes in the way special education placements are made because they say it will place financial and administrative burdens on schools.

Under the bill schools would need to take a student?s home life and cultural environment into account when making placement decisions. The measure would also speed up the process and require districts to reimburse parents for tuition payments made by parents to private schools not approved by the state within 30 days.

?It?s essentially a voucher system we have no control over,? said Jere Hochman, superintendent of Bedford Central Schools, at a recent school board meeting.

A group of 16 Westchester-Putnam school boards have approved resolutions formally opposing the bill, which Gov. Cuomo has not signed into law.

Hochman said the change could cause public funds to be channeled to private schools because parents of students who are now fully included in public schools parents might opt for the private special education program that complies with the interpretation of considering ?home environment and family background.? The language could encourage more placements of students with low-incidence disabilities whose education can be in six figures, he added.

Assemblyman Tom Abinanti (D-Greenburgh) said he opposed the bill. ?I think some of the ideas were right, but the way it?s written is all wrong,? he said.

?I think the concept of the public paying for segregated schools is contrary to the principles of American public education,? he said.

Abinanti objects to the second part of the bill as well because he said it allows parents to negotiate with school boards about where students will be placed, ?bypassing the CSE [Committee on Special Education] guidelines,? he said.

In 2007,brain cancer survivor Christine Butterfield added two new words to her vocabulary. Words she wishes she didn?t know so much about.

?Anaplastic astrocytoma
of the right frontal lobe. That was my diagnosis when I came to MD
Anderson,?she says. ?Big words I never wanted to learn ?even how to
spell!?

Snowed in
In 2007,Christine and her extended
family ?including her husband and 18-month-old daughter ?were having a
great time in Breckenridge,Colo.,about 80 miles from Denver. Then,
she started to have severe headaches and nausea. The local hospital
didn?t have imaging equipment,so they took her by ambulance to the
closest larger town for a CT scan.

?The doctor came into my room
and said,?There is a large mass on your right frontal lobe. We don?t
know what it is,so we have to get you to Denver,??she remembers. ?The
problem was the chopper was grounded,and the highway was closed because
of the weather.?

To make matters worse,it was New Year?s Eve.
Finally,the doctor found a neurosurgeon in Denver who would see
Christine,and they tracked down an ambulance willing to make the drive.

?The
doctor in Denver said it really looked like cancer,but that I needed a
biopsy to find out for sure,?Christine says. ?He recommended that I
find a place where I had support. My family lives in Houston,and MD
Anderson was the first hospital I thought of.?
High-tech treatment
As soon as Christine arrived at MD Anderson,she met Ganesh Rao,M.D.,assistant professor in the Department of Neurosurgery.

?Because of Christine?s condition,we needed to do surgery immediately,?he says. ?We performed a craniotomy and were able to remove most of the tumor.?

Rao used the high-tech Brainsuite?,an integrated neurosurgery system that allows for precise treatment of complicated tumors in sensitive areas of the brain.

Chemo,radiation ?and recovery
?After surgery,Christine had chemotherapy and radiation for three months,?says Charles Conrad,M.D.,professor in the Department of Neuro-Oncology. ?Then she had another 22 rounds of additional chemo.?

Christine was released four days after surgery,and she spent the next several weeks recuperating in Houston with family. Then,she was able to return to her home in Austin to continue chemotherapy and radiation therapy.

?I was in a weird blissful state,?Christine says. ?I thought,?I can do this;it?s just another chapter in my life.??

Singing a different tune
Christine?s life is filled with family,staying healthy and doing what she loves ?singing. She networks with Austin?s stellar musicians to find back-up professionals,then shows off her pipes with what she describes as Latin-infused standards.

?There will always be a few cancer cells in my brain,?she says. ?But right now they?re dormant,and that?s how we want them. Totally asleep.?

?Being treated at MD Anderson was the silver lining for me,?she says. ?I kept thinking,what a privilege I get to be here. These are the best and brightest,and I send them prayers of gratitude.?
